Are there are any special instructions the driver needs to be aware of?
Are there any overhanging canopies, trees, low bridges, signs etc.?
Hazard / Obstructions (please provide photographic evidence of any observations)
Are there any gradient/s (up or down) that affect the entire delivery process?
Does the width of the tail lift rest level to the floor, kerb, bay?
Is there potential for the position of the tail lift to cause a trip hazard?
Does the trailer have levelling devices / air suspention to counteract this?
Are there 3rd party parked vehicles / delivery vans that could create an obstacle?
Are there any post, barriers, fences, gates or walls that could cause an issue?
Are there any ground conditions, pot holes that cause any concern?
Does the driver have access to welfare facilities e.g. toilet/handwash?
Environment
Is the load secured adequately to account for gradients and cambers?
Are there any unguarded areas that could result in a fall from height?
Is the level of lighting adequate for the delivery window?
Are fire exit routes blocked as a result of the unloading / loading process?
Does the standard of housekeeping cause safety concerns? If so please report it.
Is there any equipment available to clear spillages during delivery?
Delivery Point / Door
Is access gained by reversing?
Is the reverse manoeuvre to reach delivery/collection point over >5 metres?
Is this a long distance (>5 metre +) blind side reversing?
Is there sufficient room to position the vehicle to complete the delivery point?
Does offloading of cages or the fridge unit affect residents due to noise nuisance?
Is all equipment used to assist with the delivery e.g. HHPT fit for purpose?
Is the distance of the tail lift to the delivery door >5 metres? Think manual handing!
